Request for Application for Enrollment

Please complete the information below and submit.  A link to the application will be emailed to the primary parent/guardian's email address to be completed and digitally signed. Once they complete the process it will be routed to the secondary parent/guardian's email (if specified) for them to digitally sign.  An application will be emailed for each student you are applying for.

If applying for middle school or high school the student MUST have their own email address in order to digitally sign the school's Code of Conduct agreement.  The application will be sent to them after completed and digitally signed by their parent/guardian(s). 

A separate email will be sent to the primary parent/guardian with a total amount due for application fees.  The email will contain a link allowing you to pay electronically via credit card or bank account (EFT).
